About Alouette Homes

Connective is partnering with Vancouver Coastal Health to operate a unique new model of care that allows people with severe and persistent mental illness and co-occurring disorders to live outside a hospital setting in a supported, longer-term home environment.

 

Alouette Homes is designed to support up to 20 people who require long-term psychiatric and social rehabilitation care in a secure group setting. The program will offer an improved quality of life with outdoor space and increased communal living and activity spaces while providing the security and staff needed for this phase of the residents’ rehabilitation.  

 

Connective is committed to a holistic, collaborative, person-centered, and trauma-informed approach that tailors our support to each resident’s unique needs. The program at Alouette is grounded in evidence-based strategies that integrates positive behavioural support with psychosocial interventions to promote recovery, resilience, and meaningful engagement in life.  

 

We are also committed to prioritizing Indigenous perspectives and providing supports that will help Indigenous residents connect with their culture and get the support they deserve. We will engage in collaborative clinical partnerships to promote traditional healing, cultural teachings, and practice, and promote understanding and respect for Indigenous culture, experience, and identity.

 

The goal of the program is to help residents meet their personal goals and needs, increase their housing stability, and minimize returns to acute care sites.

Admission Criteria

Alouette Homes supports people who are mandated under the Mental Health Act and where discharge from Tertiary care sites or into other community based settings has not been successful.

How to Apply

The local health authorities will determine who is best suited to live here based upon the criteria established under the Mental Health Act.

What is Connective’s role in relation to Alouette Homes?

Connective is partnering with VCH to operate the program. This means that Connective is responsible for co-designing this unique service delivery model within the framework of our values and for the day-to-day operations of the designated facility. Connective will staff the residence with integrated mental health workers and will be responsible for contracting allied health professionals such as clinical counselors, occupational and recreational therapists, and various other wellness facilitators. Our team will support assistance with daily living tasks and activities. Our integrated mental health workers will also explore recreational and cultural opportunities and practice psycho-social rehabilitation methods such as cognitive remediation. VCH will provide oversight to the program and appropriate medical staff support, including psychiatry, addictions medicine and primary care. 

How is this program unique? Are there others like it in the province?

Currently, there are no other options for people who have severe and persistent mental illness and co-occurring disorders to live outside a hospital setting in a supported, longer-term home environment. This evidence-based, innovative program incorporates positive behavioural support strategies and is integrated with comprehensive psychosocial interventions to promote adaptive functioning and overall well-being. To our knowledge, this new model of care program is unprecedented in the province.

How will things be different for the residents at Alouette compared to their experience in tertiary care?

Residents will be moving from a hospital setting that was never intended to be long-term into a supported, longer-term home environment. Our team will be able to build relationships and provide personalized care and enhanced supports beyond what can be provided in a hospital setting. Residents will have the opportunity to develop life skills through behavioural support and psychosocial interventions, while living in a supportive group environment.
